COVID-19 GUIDELINES

I have reviewed Inside Out Club DuPage's Covid-19 Safety Measures policy at https://www.insideoutclub.org/covid19 and agree to adhere to those guidelines. I acknowledge that the guidelines can change between registration and the event and that it is my responsibility to review and comply with the guidelines in place during the event and or program.

VIDEO & PHOTO RELEASE

I hereby expressly grant to I Support Community NFP d/b/a Inside Out Club DuPage (IOCD) and its employees, agents and assigns, the right to photograph my child(ren) and use those picture(s)/videography in connection with the promotion of IOCD and its programs. Means of promotion may include, but not limited to, the IOCD website, flyers or brochures, Facebook, Twitter, blog, LinkedIn and any other social media deemed necessary by IOCD.

TERMS OF PARTICIPATION & RELEASE OF LIABILITY

The Inside Out Club Weekend program is designed to expose kids ages 3-12 to a variety of charitable organizations working in their community to make a difference. Through this program kids will learn about items such as homelessness, hunger, community, volunteering and more. Our goal is for children who participate in the Club to develop character inside and out, a broader view of their community and understand the many ways they can contribute and give back.

We are encouraging parents to participate in the activity with their child(ren) so that they can continue the learning at home.

Inside Out Club weekend events are held at various locations on the weekend and are led by volunteers. Volunteers have not been through a background check and IOCD cannot guarantee the content of each event.

I do hereby release and forever discharge Inside Out Club DuPage, their agents, employees, and assigns, and their respective heirs, personal representatives, affiliates, successors and assigns, and any and all persons, firms or corporations liable or who might be claimed to be liable, whether or not herein named, none of whom admit any liability to the participant, but all expressly denying liability, from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, which I now have or may hereafter have, arising out of or in any way relating to any and all injuries and damages of any and every kind, to both person and property, and also any and all injuries and damages that may develop in the future, as a result of or in any way relating to the following:

Participating in the Inside Out Club weekend program including, but not limited to, attending activities and events initiated by IOCD and/or the Inside Out Club.

Furthermore, this Release shall be binding upon the participant, and his respective heirs, executors, administrators, personal representatives, successors and assigns. This Release shall be subject to and governed by the laws of the State of Illinois.

Virtual Event: By registering for a virtual event, you acknowledge, understand, and agree that this program is held on Zoom and open to other participants over whom Inside Out Club DuPage has no control. As a condition of participation, you understand that other participants may say or do inappropriate things and agree not to hold IOCD accountable for the actions or conduct of other participants.

Acceptance and inclusion are key traits that we at Inside Out Club believe in, so that every child who wants to can participate. If your family falls into one of these categories below, we are happy to waive the participation fees.

- You’re enrolled in or eligible for the Federal Free or Reduced Price Lunch program. - Your family income meets the Income Eligibility Guidelines set by the USDA Food and Nutrition Service.

- You’re enrolled in a program that aids students from low-income families, such as Upward Bound Your family receives public assistance.

- You live in federally subsidized public housing or a foster home, or you are homeless.

- Your family is experiencing financial hardships.
