During the Inside Out Club all participants are expected to exhibit appropriate behavior at all times. The following guidelines have been developed to help make programs safe and enjoyable for all participants. Please discuss these with your child(ren) before he or she participates. 1. Be respectful of all participants, staff and volunteers. 2. Follow program directions the first time asked. 3. Use appropriate language at all times. 4. Refrain from threatening or causing bodily harm to self, other participants, and/or volunteers or staff. 5. Show respect for equipment, supplies, and facilities. 6. Follow the rules in regards to behavior. If your student does not use appropriate behavior you will be emailed to talk to your student. If your student's behavior doesn't approve your student will be asked not to return and you will receive a refund for the sessions your student didn't attend. Thank you for understanding this policy.

If you request a refund 14 days or more before the start of the camp you will receive 100% of your payment back less processing fees. No refunds will be given outside of this policy as Inside Out Club will have already purchased and prepared supplies for your child(ren).

General Waiver & Release.

I, the undersigned, hereby certify that I am the parent or legal guardian of the below named Student and I have read and understand the above statements and agree to the terms and stipulations. I hereby grant my consent for the Student to participate in I Support Community NFP d/b/a Inside Out Club DuPage. I do hereby release and forever discharge I Support Community NFP d/b/a Inside Out Club DuPage, their agents, employees, volunteers, board members, and assigns, and their respective personal representatives, administrators, executors, and heirs, and any and all persons who might be claimed to be liable, whether or not herein named, (collectively, the “Releasees”), from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, (including but not limited to attorney’s fees, litigation expenses, and court costs) which I now have or may hereafter have, arising out of or in any way relating to any and all injuries, losses, and damages of any and every kind, to both person and property, and also any and all injuries, losses, and damages that may develop in the future, as a result of or in any way relating to participating in the Inside Out Club.

I further agree to indemnify and hold harmless Releasees from liability for the injury or death of any person(s) and damage to property that may result from Student’s negligent or intentional act or omission while participating in I Support Community NFP d/b/a Inside Out Club DuPage. I further agree that this Release shall be construed in accordance with the laws of the State of Illinois. If any term or provision of this Release shall be held illegal, unenforceable, or in conflict with any law governing this Release the validity of the remaining portions shall not be affected thereby. I hereby certify and represent that I have read the foregoing and fully understand the meaning and effect thereof and intend this to be legally binding.

Evaluation Release.

I Support Community NFP d/b/a Inside Out Club DuPage (IOCD) is continually working to improve the Inside Out Club. To do this they collect general, nondescript information of the child(ren)’s experience. No children will be identified by name. During Inside Out Club Live, your son/daughter will be asked to complete brief digital polls, which will ask them to share/rate their experience and identify items they learned, and upon completion, they will also be asked to draw a picture of what they learned. These activities are short and will not include your child’s name or identifying information. Your signature below indicates your consent for your child to be included in the evaluation.

Acceptance and inclusion are key traits that we at Inside Out Club believe in, so that every child who wants to can participate. If your family falls into one of these categories below, we are happy to waive the participation fees.

- You’re enrolled in or eligible for the Federal Free or Reduced Price Lunch program. - Your family income meets the Income Eligibility Guidelines set by the USDA Food and Nutrition Service.

- You’re enrolled in a program that aids students from low-income families, such as Upward Bound Your family receives public assistance.

- You live in federally subsidized public housing or a foster home, or you are homeless.

- Your family is experiencing financial hardships.
