Participant Acknowledgement:

I acknowledge that participation in the Inside Out Club activities (“Activities”) involves risks of exposure to communicable diseases, including (but not limited to) COVID-19. In addition I acknowledge that participating in these Activities may increase the risk of contracting a communicable disease, including (but not limited to) COVID-19. and that there are potential risks that may presently be unknown.

I recognize the importance of complying, and agree to fully comply with the applicable laws, policies, rules, regulations, CDC guidelines, and leader’s instructions regarding participation including the instructions in this policy pertaining to social distancing precautions, face coverings, physical separation, sanitization, non-participation if feeling ill, and nonparticipation in the event of a temperature above the CDC recommended level.

Masks:

At this time, masks are optional for all individuals, including children and volunteers, in school buildings, regardless of vaccination status.  This aligns with the recent CDC release regarding mask-wearing. Wearing a mask is strongly encouraged for all Inside Out Club leaders.

Please note that acceptable masks include multi-layer cloth, non-medical disposable, or N95. All masks must fit snugly and completely cover your nose and mouth. Please note that plastic face shields worn alone, neck gaiters, and bandanas are not acceptable. A mask with a face shield is acceptable.

If requirements change and masks are required by state or federal regulations, leaders will have masks on-hand in case a participant does not have an appropriate mask and will ask students to pull their masks up if they fall down. IOCD will contact the parents of any participant that must be asked multiple times to pull up their mask in the hour session. If that behavior persists the parents will be asked to come pick up their child.

Inside Out Club DuPage cannot warrant any facilities or equipment to be free from COVID-19 or any other communicable disease.

Vaccination, COVID-19 Screening Requirements, & Contact Tracing

Based on the statewide mandate for Illinois effective 8/30/2021, all volunteers working with children must be vaccinated. Volunteers will submit a copy of his/her vaccination card to IOCD prior to volunteering. The card will not be retained by IOCD.  IOCD will keep a record that it was provided and reviewed by IOCD.

All volunteers, staff and/or contract leaders must self-screen the morning before volunteering by being able to answer “No” to the following questions:

Are you currently experiencing any symptoms related to COVID-19?

    • Within the past 14 days, have you been in close contact with anyone diagnosed with COVID-19 or anyone who has symptoms consistent with COVID-19? ”Close contact” means being within 6 feet or less of an infected individual for a cumulative period of 15 minutes or more over a 24-hour period.
